Our group has 33 physician, and 5 mid-level providers. We use QGenda. Our former CFO, who now functions as our outside accountant, has been doing the physician scheduling for about 2 years. She spends a great deal of time with the President of the group dealing with the dozens of exceptions to the rules. She does the schedule in six month increments. It probably takes her at least 120 hours to do the schedule from start to finish; twice a year Small changes afterwards, like switches, are very easy and do not take much time. But fact gathering for vacations, plus the nuances to schedule in 16 different assignments each day, is a bear. The President of the group already gets 20 admin days and so his time is accounted for that way. We pay the CPA for this task, but she also keeps track on a separate spreadsheet to ensure equality over time for time off, so it's a high level process.
Managed Care Credentialing is done by the billing service. Facility credentialing is done by an internal person. With so many locations it takes at least 30+ hours per week with both first time and re-credentialing. There is a plan for the billing company to start doing both managed care and facility credentialing (at an additional cost). We would like that to happen.

Day to day scheduling is handled by a staff person, among other duties. She probably spends 25-30% of her time on it. She works with a Scheduling Committee that oversees staffing patterns, time off selection, etc., and consults with the chair of that committee in the event of unexpected crises.
