    Posted 10-30-2020 11:00
    Hello everyone,

Looking to see if any benchmark data can be shared regarding payment for IR call?  Do groups pay a flat hourly fee for carrying call pager, and if so, what does that look like?  How do other groups compensate your IR section for taking of call?

We are 24/7 on our DR side of the fence so the concept of 'on call' for us only really resides in IR.

    Thanks - Josh

Not a benchmark, but one data point.  Our compensation model for both DR and IR is built around the number and type of shifts worked, with the baseline being regular  M-F day shifts valued at 1.0.

We pay 0.6 for weeknight IR call, and 1.8 for a 24 hour weekend or holiday IR call.

I would guess that the right number depends on how busy your IR call is.

    Hope this helps.

    Hi David,

Thanks for your feedback.  Would you mind sharing your methodology for assigning points?  We've kicked this around in our group for a while but it hasn't gotten much traction.  With that said, we are starting to as the group continues to evolve.

    Dave,

I would also like that information. We have also discussed doing this.

It is currently very subjective for IR.  On the diagnostic side, we're looking to have enough volunteers to cover after hours shifts.  If not enough, we consider increasing the value.  If too many, we consider decreasing.  The diagnostic values have somewhat informed the IR values, though very indirectly.  We are moving down a path to be more market based on the IR rates.  A good first step would be to see at least some IR call changing hands....i.e some take more and some take less.  If that starts happening we will know we have found the right values.

Hi Joshua and Carol and David -

We provide 24/7 DR call as well.   We also have a subset of radiologists who do IR call.

The practice established a shift weight for every hour of every day.  The weight increases for work performed after 5:30 p.m., graveyard, and for weekends.  The Shift weight adds those hourly values to derive a weight for the shift.

We then appointed an ad hoc committee of 2 non-IR rads and 2 IR rads, with a partner not in those groups who was an arbiter.

The adhoc committee tallied all after hours work by DR and by IR using the shift weights.   They removed or added IR workload (call shifts) based on who they equalized out.   The practice philosophy is that all partners earn the same wage, and they work the same as measured by these weights.

IR gets a small fee < $1,000 for carrying the pager for a week.     They were removed from certain call obligations.   They also get a callback fee for cases meeting certain parameters.  Not calls when they were already assigned to work.  Callbacks when they are home, and having to drive in to cover.   Or when one after hours callback turns into 2+ cases.

Both sets of radiologists DR and IR, agree to this negotiation, and both think the other side has the better deal.   DR has to work most weekend shifts.   IR has to put up with a limited weekend shift, and the burden of carrying a pager for the week.

Been in place easily 10+ years. 35 providers.

Hope this is on point with the problem you are looking to solve.
