    Has anyone else come across this from Noridian related to levels of supervision? I thought supervision levels were the same for all settings, but is this just for IDTFs?

    This says Screening mam, 77067 requires Direct supervision level.

    Larry,

     

    The supervision levels are the same for all settings including office, IDTFs, and hospital outpatient.  However, for IDTFs they can only provide the imaging services that are on the approved list like Noridian provided.  Therefore supervision levels and rules are the same, range of services they are allowed to provide at an IDTF site are more limited.

    We are not getting paid for the new chest or abdomen codes by either Cahaba GBA (GA) or Palmetto GBA (SC).  Does anyone know if there has been any information from Medicare about the new chest and abdomen codes?  They are also not on the fee schedules for either of these MACs.

    We have Cahaba GBA in Alabama (which is the same MAC for GA) and those codes are on our fee schedule and we are being paid for them.

    We have Palmetto GBA as our MAC for Railroad claims and we are being paid as well on those codes by them.

    The 2018 RVU File shows the supervision level as a 9 for screening mammogram.  Noridian posting it on their website with direct supervision is quite different.  Which supersedes?

    The 2018 RVU File shows the supervision level as a 9 for screening mammogram.  Noridian posting it on their website with direct supervision is quite different.  Which supersedes?

    I believe the MAC's have the authority to set more stringent supervision standards for IDTF's.  Having said that, I would contact Noridian, note the variation from the CMS standard and ask if this is a typo.  If not, ask for an explanation of their rationale.

    We received word from one of our members that a local payer in his area is requiring preauthorization for all ultrasound studies.   Are others experiencing this?  If so, which payer(s) and in which state(s)?

    We had the issue here in Oregon with Moda Health Plan but that has since been overturned and no longer in effect as of 12.31.17.

    In Oregon, Moda Healthcare started requiring pre-auth for outpatient Ultrasound in April, 2017.  They ended it effective January 1, 2018.
