    Does anyone have a process in place to validate that that adequate arrangements have been made for post-procedure care (i.e. can the patient or family member handle it, or has home health been engaged), and that the ordering clinician (or another member of the patients care team) is taking responsibility for follow up after IR procedures, such as supra-pubic catheters?

     

    If so, who does the validation, what does the process look like, and how is it documented?

     

    We seem to see quite a few cases where the patient is referred for these types of procedures, and the interventional radiologist has no idea what the patients circumstances are, or whether adequate follow up arrangements have been made.
