  • 1.  Radiologists reading remotely

    Good afternoon,
    Would you have an insight into hiring per diem radiologists to read from out of state?
    Our free standing facilities have a radiologist who is licensed and credentialed in our state and has moved out of state. 
    We would like for him to read some studies.
    Is this permitted by Medicare and other insurance carriers?
    Anything else I need to know regarding this?

    As far as I know your radiologist in this case can read.

    You may have to 'split-bill' Medicare as the radiologist is not located in the same geography as the scan.

    IMHO an adjusted claim to Medicare in these instances is clumsy and doesn't make sense.

    Can anyone else assist to clarify?

    You can read remotely from any state for another state. The only difference is that you must bill the Medicare Assigned Carrier (MAC) you are sitting in. All other carriers can be billed as normal.

    Your radiologist also needs a medical license in the new state as well.

    Thank you all for your input.

    We were told by a Novitas rep that as long as we bill under the group and the radiologist is associated with the group it does not matter.

    I found this suspect, which is why I am asking the question.

    I also read something about these restrictions being suspended due to COVID, but could not verify that either.

     

    Does anyone split bill for their radiology services done in their private offices?

    Joanne,

     

    Pretty sure you received bad advice from your Novitas contact-and it does matter.  You will need to bill the P/C to the Medicare jurisdiction in which the radiologist is sitting when providing the interpretation.

     

    Commercial insurances can vary in their requirements so at least check with your major payors to confirm any special instructions.

     

    Also, some states are friendlier than others in terms of licensing requirements for physicians reading remotely-more homework to confirm that before you get too far along in the process.

     

    And finally, you should check with your medical malpractice carrier.  Teleradiology has definitely become more mainstream so this may be another simple "check the box" step, but it's worth making sure your current policy will cover the out-of-state radiologist-or whether it will need to be modified.
