  • 1.  Reimbursement for after hours

    Hello everyone,

     

                    I have a question from one of my Radiologists regarding reimbursement for after hours.

     

    Here is the question below.

     

    Some groups track after hour radiologist reads and pay them according to RVUs.

     

    Do you know of a tool we could use to tally the after hour read RVUs for individual radiologists?

     

    Plus are many groups reimbursing this way or are you using something else.

     

    Any suggestions would be appreciated,

    Donna,
    Our group created a fee schedule for after hour reads like the one with a night service. They list the cases they read on form we created and turn it in to me.Some email it some fax it. I calculate the the total and pay them at the end of the month. We just reviewed the fee schedule and updated this week after about 3 years paying he same fee.

    Donna- we do something similar to Jim's practice. The on-call radiologists have an "I need help" button on their desktops. If the volumes get too high, the radiologist presses that alert and radiologists who have volunteered for a coverage pool get a text.  If they are able, they login and help with the work list.   The practice reimburses them at 80% of the average collection rate for those exams.  We use the ED collection rate.  Its been in place for 2-3 years and works well.

    Donna,

    We call it Eat What You Kill (EWYK).  I calculated the average reimbursement per CPT code less the billing fee and about an additional 5% amount.  

    The doctors send me the information  and I pay their EWYK on a one month lag.  After they submit their info, I send them an excel spreadsheet with their particular data.   I update the reimbursement amounts every six months.

    Unfortunately, it is a manual process.  Each doctor sends me a list from PACS (without any PHI).  It has the DOS, exam name, time read, and sometimes other info (location, referring doctor...).

    I code the exams to the proper CPT and input the data onto a spreadsheet by DOS.  The doctors are paid once a month.
