  • 1.  Mammography reporting

    It has been brought to our attention by our coding team that the payors are requesting quadrant reporting in lieu of "o'clock" reporting for mammography and breast ultrasound.  Our radiologists have not had this request from other institutions they have previously worked with.  Can you please tell me if you have completely switched your reporting verbiage to remove "o'clock" reporting and replaced with quadrant reporting?

     

    I'm interested in any experiences you can share with me.  Thank you.

    We've had this discussion from just one of our facilities – the SEER Program has a chart (see page 3 at https://seer.cancer.gov/manuals/2016/AppendixC/Coding_Guidelines_Breast_2016.pdf )  that helps translate o'clock references to breast quadrants. We have not had any issues with bridging between the two, nor have we had any issues with reviews by payors. About the only thing up in the air is when something falls at 3/9/9/12 oclock or respectively is in both quadrants – depending on whom you speak to (including payor guidance),  there are instructions to code both quadrants it falls in. We have adopted that as a standard for those issues, and also put a note that comes out in freeform block 19 field with the actual position.

    We have not. All payers or specific ones. Coding Clinics has an article to allow for the clock reporting as long as an internal policy is established they just don’t define the clock positions with the quadrants.
