  • 1.  Contrast Protocol Orders

    All:

          Wanted to get some advise on contrast protocol if the contrast isn't on the order. Can our physician without notifying the treating physician go by our protocol based on the body/exam ordered?  Do we have to call the referring physician to update this each time it is missing or not listed? 

    And also if the wrong body part was ordered or wrong side was ordered can we without notifying the referring physician to change it to a left for example when they stated right hand?

    Any advise would be great, thank you!

    Diane – If my memory serves me - and the older I get I realize I do forget things – as a physician practice you need orders from the referring physician for everything that is done. The radiologist only has the "right" to alter an order in the hospital setting. So I would say that for each of the incidents you outlined that your team would need to communicate with the referring physician's practice and get an updated order.... If others have different insights please let us all know... Thanks

    Wanted to touch base on this again – can we put on our scripts an option "per protocol" for contrast procedures with a disclaimer so we can eliminate so many call backs to our referring physicians?

    Diane,

     

    When we had our imaging center, we had a checkbox on our orders forms that read, "Contrast if indicated."  This was under both the MRI and CT sections of the order form.  This way the physician has ordered contrast, but we have the option to down-code the exam if the radiologist determines that contrast is not necessary.  We had a note for physicians that wanted the exam done they way it was ordered (i.e., do not do without contrast).  It did not eliminate calls to the referring office, but it does reduce them.

    Have any groups issued ordering guides that include contrast protocols?   We're researching the idea to eliminate the call backs to referring physicians.

    We created Ordering Guides for our referring physicians that list our protocols for each exam and whether it should be w/ or w/out contrast based on the reason for the exam.  These have been well received by the referring physicians, especially general practice providers who aren't always sure whether an exam should include contrast or not.  Also, some insurances will only authorize the specific CPT code (vs. the family of codes) so if they are getting prior authorization they would need to know ahead of time if the exam is with contrast or not.  ​
