We worked out our old (in-house) AR for 90 days and then turned remaining old AR to a local agency that has a third-party billing unit and collected on the turn under a percentage arrangement.  I'd be happy to refer them to you if interested.  The end result was very good, both PR and collections, but it was not with some folks unhappy to have their active account turned over to a "collection agency," but once explained it was a third-party billing arrangement it was understood and good relations maintained.
