COVID-19 CMS Advanced Payments

    RBMA continues to monitor CMS policy and rules around COVID-19 initiatives.  One is the Medicare Advanced Payments program. 

    The latest update is  from a colleagues at Zotec Partners  is that there is no guidance on MACS distributing these payments as yet.  It seems that HHS is directly transferring funds to providers for this first wave (electronically, e.g., directly to tax id numbers). Providers may be notified soon on this.   Allocation of funds is based on amount received from Medicare under fee-for-service (not MA) by provider from Medicare in 2019.  

    More updates as we receive information from Congressional staff....

    I have read that 10.25% interest will begin accruing on day 241 for any remaining amount that has not been recouped by CMS.  However, this is not mentioned on the CMS Fact Sheet and I am trying to determine the validity of this.  Anyone have any additional information on this?

    Our group submitted a request for the advance payment on 4/1/20 (via email) and received the funds on 4/8/20. The MAC did send an email notifying us of the payment amount but it arrived a day after we received the funds. Surprisingly quick turnaround.

    We received our Advanced Payment Medicare deposit over the weekend, but only approximately half of what we had requested based upon calculated prior Noridian payments.  We haven't received any notification from Noridian as of today.  Have other groups received less than requested?  Did you all eventually receive an email with details?

    Are you sure it wasn't the Provider Relief Fund payment that so many other practices received this weekend? (It comes across as an ACH with "HHSPAYMENT" or something similar to that.

    We received both this weekend.  One "HHS Stimulus", one "Noridian" with no details but way too large for anything but APM.

    The Provider Relief website (https://www.hhs.gov/provider-relief/index.html indicates that "Within 30 days of receiving the payment, providers must sign an attestation confirming receipt of the funds and agreeing to the terms and conditions of payment. The portal for signing the attestation will be open the week of April 13, 2020, and will be linked on this page."  

     

    Has anyone been able to access the attestation and, if so, could you provide a link to it please?

    Hi Larry

    I don't think the attestation is available yet.  I'm sure there will be a link on that page when it is.

    It is worth reviewing the Terms and Conditions carefully, especially the limitation on paying salaries above federal executive level II ($197,300) and the accounting requirements outlined at 45 CFR § 75.302 before you start doing anything with those funds.  I'm guessing there will be auditing at some point in the future.

    If you received the payment in your bank account by ACH and you do not qualify for the payment
    call your business banker and refuse the entry with code R-23. There is nothing on the portal's  attestation for returning the funds. I called the CARES provider relief number (866-569-3522) and got this information.

    ACH Return Code R23 – Credit Entry Refused by Receiver. Description: Receiver returned Entry because, for instance, minimum or exact amount not remitted. Any credit Entry that is refused by the Receiver may be returned by the RDFI.

    Yes...one TIN was fine, one was rejected as you described.   Call to support was left on hold for 30+, then disconnected.  Will try again, but how in the heck can the TIN not be identified?   A bit frustrating, but thankful none the less.

    It also goes on to say that if no attestation is signed and the provider keeps the money, then the provider is bound by the attestation.  This means the provider, at the very least, will not balance bill any COVID patients for care.  Since most carriers are sending out notices that they will be paying COVID claims with no patient out of pocket, that provision may not be worrisome.

    It has been fairly quiet but I am getting them.  You might check your set up.  For some reason I was dropped of the coding forum for about 2 months before I realized it.  I didn't terminate it so I don't know why it happened.

    Wendy,

    We received our Healthcare Providers Lost Revenue Grant last week and, while others received an email about it, we did not.  Because I had applied for the Advanced Payments a few days earlier, that's what I originally thought it was, and I was surprised because it was about 1/3 of what I had expected. 

     

    Today I received an email that the Medicare Advance Payment will be hitting our account tomorrow.  The amount was close to what I expected.  Very confusing when money shows up without any explanation.  I definitely appreciate the added cash flow, however!

    I know things are evolving quickly with lots of conflicting information, but unfortunately what your colleague gave you is misleading.  This article from Healthcare Administrative Partners talks about the money from HHS and correctly says that it is not connected with the Medicare Advanced Payments.  Two separate things, and practices can get both.
