Looking for a price-range for filling out SDI paperwork. I have a Nuclear Medicine physician in our practice who sees patients in consult and follows them after treatment for thyroid cancer with office visits. Sometimes the patients need extended time off work to recover from treatment, and she is asked to fill out the State Disability forms. The forms can be 10 pages of questions. We've decided to charge for this extra physician time, so I'm looking for input on what others may be charging for this service? Thanks in advance.
