Our breast practice does diagnostic ultrasounds.   Our patients must have an order for this service.  I am not aware of a screening ultrasound.  Hope this helps.

There is no unique CPT code for “screening” breast ultrasound. Screening would use the complete breast ultrasound code (76641) rather than the “limited” (76642). ICD code would need to be appropriate. Typical code for screening ultrasound because of dense mammogram is R92.2 (“inconclusive mammogram” )

An order is needed for all ultrasounds.

Medicare pays but applies to deductible and copay (ie not completely paid like a preventative “screening” service)
