  • 1.  Billing for Fiducial Markers

    Good morning,

    I'm hoping to speak with anyone that has some expertise in billing fiducial markers placed in the kidney for radiation therapy guidance.  I want to confirm that cpt code 49411 is the correct procedure for this service and if ct guidance as add on code is allowable.  Trying to understand financial impact if we start to perform these.  We do not currently.

    Yes, you're correct. For the placement of interstitial devices such as fiducial markers for radiation therapy guidance, CPT code 49411 is used for the procedure in the intraabdominal, intrapelvic (except prostate), and/or retroperitoneum areas. Additionally, based on the instructions within CPT, if CT guidance is used for the placement of these markers, it is appropriate to bill it separately with code 77012 for CT guidance. Therefore, in your scenario, billing would involve both 49411 for the placement of the fiducial markers and 77012 for the CT guidance used during the procedure.

    Greetings all,

     

    I do agree that the most appropriate CPT codes are 49411for the placement plus 77012 for the CT guidance.  Depending on the site of service, there may be other codes available to report.  For example, the CPT® manual states that reporting the supply of the device is separate.  In the office of OBL setting, it would be appropriate for the provider to report the markers separately.
