JW/JZ Modifiers for IDTFs

    Good afternoon!

    I've read lots of material about reporting the waste but i've heard conflicting information on contrast material for IDTFs. Is contrast exlcuded or do we need to report waste?

    Hi Beth, our imaging center is owned by the hospital but it's not an IDTF.  Does this rule for contrast waste still apply?

    Oops - I read your question incorrectly!

    The hospital requirements are for drugs with a status indicator of G or K.

    I have seen policies out there for commercial payors in regard to drug waste. They align with the CMS policy.

    So, is it best to do it for all insurances then? 

    Beth- Medicare replacements as well? 

    Taleesha- I belive you on this, as this is the case for lots of denials and such, some commercial's follow Medicare.

    I typically adopt MC policies for all carriers and deal with their proprietary policies as appropriate.

    Regardless of how you implement, all payors, Medicare only, maps only, policies such as this are sure to cause issues.

    RBMA has been involved in discussions with CMS regarding this issue.  On September 7thy RBMA along with the ACR, RADNet and Rayus Radiology met with several CMS officials regarding the modifier requirements, the difficulty in administering the requirement and a request to waive the requirement prior to the 10/21/23 enforcement date.  It was a good discussion and RBMA also cited this issue in the MPFS Proposed Rule Comment Letter submitted yesterday.  Attached is the letter that was sent to CMS from the parties above regarding the issue.
