    We have an IDTF that we are considering this for.  It will be helpful, even if it only extends through the end of the year.  

    The IDTF owner thinks that there needs to be a physician extender present when the physician is supervising remotely.  I don't think that's the case.  As I understand it, there are 2 additional ways to achieve Direct Supervision other than through a radiologist on site. The first is to have an NP or other form of approved physician extender on site supervising the study. The other way is to have the physician supervise the study via virtual presence rather than physical presence. Virtual presence, to my understanding, does not require a physician extender on site.  

    Please advise.  Thank you.

    I heard a rumor last week that Medicare is extending the virtual supervision for an additional 2 years.  I am trying to confirm.  I have not seen anything in writing.

    The 2024 Proposed Rule released last week extends the allowance for direct supervision via real-time audio/video through 2024.  It's a proposed rule, so not a done deal yet.  CMS solicits comments on what happens after that.

    In the proposed Medicare Physician Fee Schedule Rule for 2024 released last week by CMS, the Medicare agency has proposed extending permission to use real-time audio and video presence and immediate availability via telecommunications for the direct supervision requirements for Level 2 diagnostic tests through December 31, 2024. CMS is also soliciting comments on whether to extending such virtual supervision beyond December 31, 2024.

     

    If such change were not adopted, virtual supervision would be scheduled to end on December 31, 2023.
