  • 1.  Deductibles and no shows

    ​Good morning everyone,

    Has any practice collected on deductibles at the time of service or a portion of the deductible.  If so how is that going.

    Are the patients cooperative in paying?

    Also, are any practices charging for no shows?

    I have not come across a practice to charge "no show" fees for radiology.

    No shows are tough...part of the business or radiology.    Look for repeat offenders would be my take.  In regard to deductibles, we simply implemented a Point Of Service (POS) payment program by modality and kept the fees nominal.    We trained staff not to say "co-pay" and there are exclusions by payor (Medicaid for example).   The POS payment notification starts with scheduling so patients aren't surprised and it's not mandatory; no awkward confrontation at check-in if they can't may the POS payment.    A softer approach has paid sizeable dividends for our practice.  You may already be doing this, not sure, but no reference to deductibles – difficult to be specific.   One quick note....for any non-believers in taking POS payments due to potential refund issues, it really hasn't panned out as such over the last few years - probably because the amount we're collecting per modality is minimal, reasonable, and non-confrontational.

    For PET patients, we calculate out of pocket expenses for each insured patient.   We then call then 3-4 days out to discuss this with the patient.   In 2019 we collected all out of pocket expenses prior to service including any previous balance on account.  This helped our AR quite a bit.   Patients at the beginning of the year are especially difficult  because of understanding their deductibles along with their out of pocket expenses.  We explain and work with them.   In 2020 we implemented a change that requires any insured patient that has an out of pocket expense of $1000 or more to pay a $1000 deposit at time of service because we were trying to appease the referring physicians that complained we  were not seeing patients because of money.   This change will definitely impact our AR negatively.    As for no shows, if we have a patient no show three times, we will contact the patient and referring and let them know we will not reschedule patient.   If a patient cancels three times we let the referring know and do not reschedule them unless there is a legitimate circumstance presented.
