Denial - Physical Address Needed?

    We've started to see some claims denied due to not having a physical address for the Billing Provider. (We do provide the physical address of where the service was provided in box 32, but we use our lockbox address in box 33.) Since we have shut down our billing office in favor of a work-from-home model, I'm not sure how to address this (excuse the inadvertent pun). We are hospital based and really do not want to provide the hospital address as the address for payments. Anyone had this issue recently? In both cases I've seen lately, the claims were going to a state Medicaid program (different states).
