    Good Afternoon,
I was wanting to get some feed back on how common is it for a private radiology group to contract with a hospital for services, and still do their own billing. All the while receiving a stipend from the hospital for their services? Alternatively, how common is it for a radiology group to contract with a hospital and have the hospital do their billing and then the hospital would compensate the radiology group?

    Thank you

    Hi Melissa

If I'm interpreting your question correctly, I think the vast majority of private hospital based groups do their own billing.  Stipends are much less common than they once were, but there are still a significant number, especially when payor mix or call obligations are economically challenging.

It's not unheard of for hospitals to bill and pay the radiology group...I believe it was actually quite common in the early days of the specialty.  However, I think it's fairly uncommon now because radiologists generally don't want to give a cut of the professional fee to the hospital and radiologists are likely to be much more interested in negotiating professional fees with payors than a hospital.

This might make for an interesting RBMA survey.

    Melissa,
Both my current group and my last group had a stipend and did our own billing.

Melissa – There are any number of approaches that are utilized. The ones you outlined as well as others that do not require employment of the radiologists by the hospital. Of real interest is why the question. Understanding the reason behind the questions can allow you to develop a better response or position.....

    Good afternoon Melissa,

Reflecting to what Dave indicated, it is fairly common if not the norm for private rad groups to contract with hospitals yet perform their own billing. The times where global billing would be more common is with ambulatory imaging.  The model would be for the hospital to bill global and pay the rad group a previously agree to % of net revenue.  Another way of getting around stipends is negotiating a performance based  bonus (never volume driven)  on agreed to KPIs.  Such as % of subspecialty reads, TAT, etc.

Hope this helps...email me if you need additional clarification.  Thanks...
