One of our coders had this to say to answer your question.

Elastography is to be billed based upon the imaging modality utilized to capture imaging.  For ultrasound elastography of breast, you would  bill CPT 76982 for the first lesion targeted.  If additional lesions are targeted, add CPT 76983 for each additional target lesion.  MRI elastography is billed with CPT 76391 with unit of 1 regardless of the number of lesions imaged.  For modifier usage, global billing no modifiers would be applied to each CPT.  For technical component billing, TC modifier is applied for each CPT. For professional component billing, 26 modifier for each CPT billed.  Please note modifier 50 bilateral procedure is not applicable to ultrasound nor MRI elastography. In addition, CPT 76983 each additional ultrasound elastography on targeted lesion(s) is currently limited to 2 units per date of service due to Medically Unlikely Edits (MUE).

Please note that 76982 and 76983 are for targeted lesions.  76981 is used for organ elastography.

We see this used most often with liver US.
