Please note that 76982 and 76983 are for targeted lesions. 76981 is used for organ elastography.
We see this used most often with liver US.

One of our coding experts had this to say to answer your question:
Elastrography is to be billed based upon the imaging modality utilized to capture imaging. For ultrasound elastrography of breast you would bill CPT 76982 for the first lesion targeted. If additional lesions are targeted add CPT 76983 for each additional target lesion. MRI elastrography is billed with CPT 76391 with unit of 1 regardless of the number of lesions imaged. For modifier usage global billing TC modifier would be applied to each CPT and professional billing 26 modifier for each CPT billed. Please note modifier 50 bilateral procedure is not applicable to ultrasound nor MRI elastrography. In addition CPT 76983 each additional ultrasound elastrography on targeted lesion(s) is currently limited to 2 units per date of service due to Medically Unlikely Edits (MUE).
