    Views/protocol/order concerns

    1) One of our radiologists would like to change a wrist protocol to include a scaphoid view on any patients with an injury or trauma. This would make the wrist protocol 4 views, but the CPT code remains a 73110 (minimum of 3 views). My question is if the ordering physician sends over the order and it has 3 views on it or has the views listed out, would we need to make contact with the office to get the ok for the 4th view.  I am not concerned for the hospital settings as there is more leeway there and technically there is no up-charge as the CPT code stays the same. 

    2) Pediatric patients are sent over with order for hips 2 views bilat with or w/o pelvis.  To decrease radiation the protocol is actually to do 2 view pelvis including a frog leg.  I am assuming each of these orders need a call to get a corrected order on file as well.

    It is so difficult to get the ordering physicians to select the correct procedures so that interfaces with PAC's happens without intervention, that this will be an additional battle as well.

    The main concern would be for the office based clinics.
