Jennifer - Sorry for delayed reply. Not sure our situation applies in your environment. We have an office with 7 US rooms. It uses EPIC for the EMR.
Process is when orders come in, the techs protocol. They work with the schedulers to get the order altered or routed to correct service (vascular, breast center in some cases, etc). If there is a trend with some offices, marketing schedules an educational lunch with that office. We created an US protocol guide and have provided that when needed to differentiate between the various types of orders. Schedulers can use some processes in EPIC that ease the ordering docs process to re-order the exam. I too would be interested in whether others have a process they find has yielded long term(or even short term) benefit.

Ultrasound orders seem to be a constant source of frustration in our practice. It seems like we are constantly reaching out to referring provider offices to request changes. How do other practices handle situations where there is a finding that would warrant additional ultrasound exams? For example lets say a sonographer is doing an exam for fetal growth and we find that the baby is small for gestational age. We would like to be able to automatically add on a doppler and biophysical profile, but right now our practice is not to do that if it isn't on the order. We encourage referring providers to list these "as needed" on orders but that seems increasingly difficult to get them to do.
