Here's a link to a survey from 2017. I doubt there has been too much change:
https://ehrintelligence.com/news/top-10-ambulatory-ehr-vendors-by-physician-practice-ehr-implementationsThere are some regional physician EHR players, so the above may not be very complete for your area.
Not sure whether or not this would hold true for physician practices, but on the hospital side I know you would not find a 1 to 1 relationship between EHR's and CDSM's. Our hospitals all considered multiple CDSM's, and the large university system where my daughter works on the Epic team actually started implementing one CDSM, and then changed to another when the first proved not to be user friendly.
I think you might be able to ask your major referring practices which CDSM they use and whether there would be any exceptions, and perhaps default to that code to avoid having to ask on every order. I doubt that the EHR will be relevant.
Good luck!

Subject: Does Anyone have a Crosswalk of EMR systems and their CDSM?
As we put our AUC processes in place, I am looking for a list of the most popular EMR systems (used by the most practices) along with which CDSM (Clinical Decision Support Mechanism) it uses.
Once we know which EMR system uses which CDSM, it will be easier to know the G-code to use. Thank you for your suggestions or resources!
