    We also have Cahaba GBA and Palmetto GBA as our MAC for Railroad claims (until the transition on Feb 26). We are seeing the new codes on our fee schedule.

    I'm sorry, you are correct, they are listed on the Palmetto fee schedule but not on the Cahaba GBA for GA locale 99.

    Thank you, Christie.  I took a look at the EOBs myself and the reason these are denying is stated as "4: The procedure code is inconsistent with the modifier used or a required modifier is missing."  The only modifier used is 26 and there are no other services billed for this patient so not sure what else would be needed.  Any ideas?

    Did the patient have a chest and abdomen? Or which code was billed?

    The ones I'm looking at currently are just chest codes 71045 & 71046.
