I have been asked to see if one of our group could buy a condo on a US territory, i.e. US Virgin Islands, Puerto Rico or Guam, and staff with a radiologist or radiologists that are licensed in our state,  and be able to read remotely from that location, and be able to bill and be reimbursed by the providers. Any experience on this would be appreciated.

I don' think this would work in a  foreign country but not sure about a US territory.

If the radiologist was a W2 employee of the group, it would depend whether Medicare serves the population.  They are US territories, so maybe they would, but that would be step 1.  If so, the rad group could add the address as a practice location to their group, and billing would have to be split, technical and professional component, billing technical to the state in which the study was performed and professional to Medicare in that territory.

Legal opinion would be good, and territory reimbursement rates obtained prior to moving forward.
