PA performed procedures in IR

  • 1.  PA performed procedures in IR

    Hello,

    Does anyone have their PA's performing minor procedures in hospital IR (ex. modfied barium swallows, paracentesis)?  If so, who dictates the report and I am assuming you bill under the PA's NPI?

    We have both an NP and PA and both dictate their work, which is then signed off by a supervising IR doctor for all reports. The services are billed under the NP and PA names/NPI.

    Hello Andrew,

    We do have our PAs perform such procedures and do indeed charge under their NPI (as this is a reduced rate, compared to the radiologists).

    1. Have you found that the reimbursement for the PA is generally 80-85% of the physician payment?
    2. Does the PA add a supervision statement to every final report?

    Re the question asked by Mr. Kazzaz:

    1. Does the PA add a supervision statement to every final report?
    Please note that PA's  and NPs are subject to general supervision of a physician. And PAs and NPs are not permitted to supervise technologists.  They may "perform" those tests and procedures that are within the scope of their licensure and hospital privileges, but they may not "supervise."  That may consistent with the question above, but just wanted to add that clarification about the act of supervision. So if a statement is added to to the report, it would not that the PA or NP was under the general supervision of a particular radiologist.

    I am aware that PAs & NPs are not permitted to supervise technologists. My question below re supervision was in the context of a PA performing a procedure under the supervision of a physician. I was curious to know if a statement to that effect was added to their report.

    Zaid,

    I assumed you understood, but wanted it to be clear to the forum. As I said, if a statement is added to to the report, it would note that the PA or NP was under the general supervision of the particular radiologist responsible to to so to maintain the practitioner's licensure. That would not be another physician who is somehow participating or observing, necessarily.  Since this is a physician procedure performed and billed solely by the PA or NP, I don't see any reason to reference any physician other than the general supervising physician. And for tests performed personally by PAs or NPs who may not supervise technologists in connection with the test in conformance with the supervision rules at 42 CFR 410.32 , I see no reasons for the interpretive report to reference the PA or NP at all.

    Tom, is a note stating general supervision by Dr. X required in the PA's note?  Does the Dr. have to co-sign any interpretation?

    Donna, I'd answer "no" to both. It's not non-compliant to do so, but unnecessary. For tests, my understanding is that only the radiologist performs and bills for the interpretation of the tests, since PAs and NPs do not have hospital privileges to interpret diagnostic tests.
